In-Home Support Services

In-Home services for children and adults with developmental disabilities in their homes and the community, children as young as 6 years old. This is a person-centered program of care that meets the physical, emotional, health, and safety needs of the individuals served. Our support professionals,

Direct Support Professionals (DSPs) support and assist each individual with

 
  • personal hygiene skills 
  • meal planning & dietary support
  • medication management
 
  • community integration​
 
  • skill building
 
  • positive behavioral management/ support
     
  • recreation/education
 
  • social skills development/ social interaction


   These services are 1:1 supports  
Independent Living Services

Independent Living Supports are provided to adults with developmental disabilities who have the BI (Building Independence) waiver, who live in their own homes and need minimal, targeted assistance to maintain independence and stability in the community.

 

      What We Provide

  • Person-centered services based on the Individual Support Plan (ISP)

  • Skill-building for independent living (cooking, cleaning, organization)

  • Support with budgeting, bill management, and financial awareness

  • Assistance with scheduling and attending appointments

  • Health & safety monitoring

  • Community engagement and social integration

  • Flexible, drop-in support tailored to individual needs

 

 

      Service Model

  • Individuals live independently in their own homes or apartments

  • Services are intermittent and based on assessed need

  • Focus on teaching and maintaining skills, not long-term dependence

  • Encourages choice, dignity, and autonomy

 

 

      Who Is a Good Fit

    Individuals who:

  • Are enrolled in a Virginia DD Waiver

  • Live independently or are transitioning to independent housing

  • Require minimal to moderate support to remain stable

  • Benefit from ongoing skill development and monitoring

  • Do not require 24-hour supervision

 

 

Supported Living Program

We provide individualized Supported Living services designed to help adults with developmental disabilities live independently in their homes whether it's leased, owned or a family residence while receiving the support they need to thrive.

Individuals must have the Community Living (CL) waiver or the Family & Individual Support (FIS) waiver.

 

      What We Offer

  • Designed for individuals who do not require 24-hour supervision but still need consistent support
